YOUR VOICE MATTERS!

You may have received a survey from the Centers for Medicare & Medicaid Services (CMS) this summer about your experience with Blue Advantage.

If you receive one, we hope you'll take a few minutes to share your honest feedback based on your experience with us.

CMS uses your feedback to rate how well we served you and met your needs, which in turn determines our ability to continue serving you in the future.

We made enhancements in 2025 to better serve you, we hope we met your expectations and request that you share your feedback with CMS to help us continue meeting your needs in the future.

Surveys will arrive in the following forms:

  • Mail surveys in an envelope with a CMS logo in the top left corner, that includes a QR code so you can complete it easily from your smartphone. To use the QR code, open your phone camera, hover over the code and tap the link that appears.

  • Phone calls from a CMS-authorized vendor. Your phone display should show PRESS GANEY LLC (the vendor's name), but this may differ depending on your phone service carrier.
