Disease Management Programs for Members

WHAT ARE DISEASE MANAGEMENT PROGRAMS?

Blue Advantage’s Disease Management Programs are educational programs for members with chronic health conditions. The purpose of the programs is to help members better self-manage their condition. Blue Advantage offers the following Disease Management Programs:

• COPD
• Diabetes

HOW DO THE PROGRAMS WORK?

Participating members receive phone calls from Disease Management Pharmacists who discuss and provide education on all aspects of care, including required labs, exams, and vaccinations in addition to a full medication review. After a brief health survey, an action plan will be created to help you achieve your health goals. You may opt-out of the programs at any time during enrollment.

WHY SHOULD YOU PARTICIPATE?

• Available at no cost to Blue Advantage members
• Conducted conveniently over the phone in the comfort of your home
• You will receive educational mailings, health reminders, and information regarding plan benefits specific to your disease state
• A full medication review is provided and Pharmacist Disease Managers are available to answer your questions throughout your enrollment
